3rd Annual Outdoor Kids Recreation & Acitivity Summer Day Camp

 Ages/Dates/Times

Ages 6-8: *May 28–June 1. *Camp will start Holiday Monday.

Ages 9-11: June 4– June 8. 9 a.m. – 3:30 p.m. Monday-Thursday | Friday, 1p.m. dismissal.

Enrollment:

Due to staffing ratios of 15-1 and facility use policies, camp is limited to 180 participants on a *first come, first served basis. In order to fully enroll a child, you must: *The first 180 applications with fully completed enrollment forms, surveys, and liability waivers, submitted by May 4, by 5:00 p.m. will be admitted.

1. Fill out an enrollment form – located ONLY in Forest E. Wyatt Center;

2. Sign an informed consent and complete a lifestyle survey;

3. Sign a youth release of liability form;

4. Pay a $25 per youth registration fee; (CASH or CHECK ONLY) and

5. Drop off all registration materials to camp headquarters at Forest E. Wyatt Center with the division secretary.
