Rehearsal/Studio Room Reservation/Policy Form (Students)

Available Hours:   Monday-Friday 6:00pm-9:00pm OR 9:00pm-12:00am

(Please note that if you need a day and time not listed above, please make your request in the Additional Comment Field.)

Students reserving DMI rehearsal rooms and/or studio space need to be aware that once they are in the building, they are not allowed to prop open a room or outside door. This causes security concerns for DSU Police who have to make a second round of door checks.

Student making rehearsal room reservation MUST be either currently enrolled in DMI coursework and MUST be present at rehearsal.  All participants must be listed on the reservation form. Equipment request may addressed in the additional comment box.

Band leader/member/ student must complete and submit the information below to DMI 24 hours or more in advance of rehearsal to request room reservation.  Requests submitted after 5pm on Friday will not be processed until the following business day.

Please be reminded that DMI policy mandates student of record must not leave the studio unattended at any time, no persons should be involved in your session that have not been listed on the form, and that as the student of record, you are responsible for the proper use and security of all equipment.
